The 5 whys technique is an effective tool whose primary goal is to find the exact reason of a problem by asking a series of ‘why’ questions. As this reveals that the error might not be what it seems as usually human error is not accounted for in which asking why could reveal it. This – in turn – helps us better find the root cause. It allows the process to be collaborative and a confidence builder within a team that can help prevent future or recurring problems.
